I am a fourth year medical student from Boston University working at Kisoro District Hospital for a year. We have an 8 yo patient who is followed by our chronic care clinic for R sided heart failure. She recently received an echo at Mulago hospital showing a severely dilated RV w/ severe systolic dysfunction, as well as severe pulmonic stenosis. The echo report says there was no ASD or VSD appreciated. By history and physical exam, she seems to be suffering from tetralogy of Fallot - she has cyanotic spells, relieved with squatting. She sats very poorly while standing (60s) and improves with squatting (high 80s). She needs surgical intervention. We have some funds for travel for this patient and her father.
